Karyotype 46, XY. lsh der(4)t(4; 5)(p16. 3; q35, 3)mat(D4S96- ), 22q11. 2(TUPLE- 1x2)
Conventional cytogenetic analysis showed a normal male karyotype, however FISH studies using the Oncor probe D4S96 specific for the Wolf Hirschom locus have shown a deletion to be present (at 4p16. 3) which was due to the inheritance of a derivative chromosome 4 following a reciprocal translocation between the telomeric regions of the short arms of a chromosome 4 and the long arms of a chromosome 5 in the patient's mother.
This finding confirms the clinical diagnosis of Wolf- Hirschom syndrome.
As this rearrangement carries a reproductive risk, genetically related individuals should be karyotyped in an appropriate order,
A separate FISH study using the VYSIS probe TUPLE- 1 specific for the Di George region on chromosome 22 indicated this locus was intact.

Karyotype 46, XX. ish t(4; 5)(p16. 3; q35. 3)(D4S96- ;D4S96+ )
Conventional cytogenetics showed a normal female karyotype, however FISH analysis using the Oncor probe D4S96 specific for the Wolf- Hirschorn locus on chromosome 4 showed that the D4S96 locus (region p16. 3) had been translocated to the telomeric region of the long arm
of one chromosome 5. This indicates a reciprocal translocation between the telomeric regions
of the short arms of chromosome 4 and the long arms of chromosome 5. .
This finding would not be expected to have a phenotypic effect on Vitaliya but would provide an explanation for her son's phenotype which is that of Wolf- Hirschorn syndrome, having received only the derivative chromosome 4,
Prenatal diagnosis should be offered in any future pregnancy,
As this rearrangement carries a reproductive risk, genetically related individuals should be karyotyped in an appropriate order.
